ELIGIBILITY

  • Adults, age 18 years and older, who have been diagnosed with a year or less to live. A diagnosis can be cancer, ALS, or any other illness.
     

  • Residents of Alaska who have a family member that is diagnosed with a year or less to live. We will provide travel assistance, if needed, for these residents to visit their loved one.


DREAM GRANTING GUIDELINES
Listed below are the following restrictions, limitations, and guidelines that apply.

  • Recipient must be an adult with a terminal illness – We define adult as someone 18 years of age and over. We define terminal as someone with less than one year to live. The recipient’s treating physician must sign a medical form verifying the health of the recipient.
     

  • The dream must be the dream of the recipient – Recipient must be able to communicate and comprehend the dream experience.
     

  • Participants in the dream must be the immediate family of the recipient – We define immediate family as spouse or significant other, children under the age of 18 living in same household, or father, mother, and siblings under the age of 18 living in the same household.
     

  • The dream must not be a surprise – We do not want to risk violating the privacy of the recipient.
     

  • The dream must be legal – The dream must not violate any laws.
     

  • The dream must be safe - The nature of the dream must not present any danger to the dream recipient or any sponsored participants. This includes travel dreams that may be detrimental to one’s health.
     

  • The dream must take place in the United States – The dream must take place in the United States and cannot include requests for visas or cruises.


The Dream Lives On CANNOT grant the following types of requests:

  • For cash, automobiles, RV rentals, real estate, property, or home improvements

  • To provide legal assistance or medical treatments

  • For assistance to pay bills (including medical expenses)

  • For visas, cruises, or travel outside of the U.S.

  • For those who are not residents of the United States


Dream Recipients are encouraged to make realistic choices. Please make choices that will fulfill a dream from the heart.

All dreams are subject to approval by The Dream Lives On organization.

The Dream Agreement is required by law and needs to be completed by every recipient. Please complete and return with completed application to address listed below.
